One of the greatest misconceptions about dementia is that it begins in our 70s or 80s.

Supporting stable blood sugar, maintaining muscle mass, and ensuring adequate caloric intake become increasingly important during this time.

While elevated BMI and insulin resistance are risk factors for cognitive decline, chronic under-fueling is a nutritional setting that is not often described. Restrictive diets, prolonged fasting, or appetite suppression with GLPs not nutritionally corrected, can send the body signals that resources are scarce. Research has even demonstrated temporary reductions in brain volume following endurance events like marathons. While these changes can recover with adequate nutrition, chronically under-fueling may have lasting consequences for brain health.

Nutrients That Matter

Several nutritional factors repeatedly emerge in the research on healthy cognitive aging:

  • Omega-3 fatty acids support healthy cell membranes, circulation, and help regulate inflammation. They also help us retain lean mass.

  • B vitamins help regulate homocysteine, an amino acid that, when elevated, has been associated with cognitive decline. Some individuals benefit from methylated forms of these vitamins based on their genetics and laboratory findings.

  • Iron is essential for oxygen delivery and brain energy production. Both deficiency and excess can impair cognitive function, making individualized assessment important.

  • Vitamin D supports immune function, healthy inflammation, and overall brain health.

  • Creatine is emerging as one of the most exciting nutrients for brain health. Creatine serves as an important energy reserve for the brain, helping regenerate ATP—the fuel every brain cell depends on. Research suggests creatine may improve memory, processing speed, mental fatigue, and cognitive resilience, particularly during periods of increased metabolic demand such as menopause, aging, or sleep deprivation.

A typical maintenance dose is 5 grams daily. Interestingly, research has demonstrated that during periods of sleep deprivation, increasing to 10 grams daily may better support cognitive performance by meeting the brain’s increased energy demands. We encourage our patients to split that into two 5 gr doses.

Sleep Is Brain Medicine

Many women notice sleep becomes more fragmented during perimenopause.

Hot flashes and night sweats don’t simply affect comfort—they disrupt restorative sleep, influence mood, and impair verbal memory. Data demonstrates these disruptions affect cognitive performance the following day.

Addressing vasomotor symptoms is about far more than improving sleep quality. It may be an important component of protecting long-term brain health. HRT is one tool, of many, we employ, but nutrients and herbal regimens can also meaningfully inform.

Build Cognitive Reserve

The concept of cognitive reserve refers to the brain’s ability to compensate for age-related changes.

Research suggests we can continue building this reserve throughout life by:

  • Continuing to learn new skills

  • Reading and pursuing lifelong education

  • Spending time outdoors and maintaining meaningful social connection

Genetics Are Not Destiny

Genes such as APOE4 can increase the risk of Alzheimer’s disease, but they are best viewed as risk amplifiers—not guarantees.

Lifestyle factors including nutrition, sleep, exercise, inflammation, stress, and metabolic health all influence whether that genetic predisposition becomes clinically significant.
